Our Services
Every visit in our practice is conducted by a Licensed Physician or Nurse Practitioner. At your initial visit, you and your provider will develop an individualized plan of care to meet your specific needs. The plan will be enacted by your provider, who will perform and coordinate relevant services.

Transitional Care
When you leave the hospital following treatment of an acute condition, you may require ongoing care for a chronic medical condition. Your care may necessitate special equipment or services. In these situations, Argyle Health Services will act as your Primary Care Provider during this time of transition. Our Providers will visit our patients in their home setting, bridging the gap, while managing their medications and overseeing their Home Health or Rehabilitation needs. Once our patients have completed their additional Skilled Needs our Providers will Transition the patients to their original Primary Care Provider or move the patient into our Chronic Managed Care group.
What Is the Purpose of Transitional Care? Transitional care is a short-term skilled need for medically complex patients transitioning from the hospital to home, or from one care setting to another. The goal of our Transitional Providers is to assist in recovery by providing primary care needs directly in the home to help you or your family member regain a certain level of independence.

- Physical therapy
- Occupational therapy
- Wound care
- Fall prevention
- High-flow oxygen and close respiratory therapy monitoring
